Photo ID Badge Request

Photo ID badges are required to be worn by all employees who work in Town Hall Administrative offices and related field positions such as our DPW staff members. Please use this form to request a badge. You will be required to upload a head shot with a white background, like the sample shown here. This can be against a white wall or door. If you need help, please check with your division admin who can then engage HR as necessary.

Please note this is not the key fob for printing and building access. Key fobs can be requested through the DPW administrative offices. Thank you.
